When renting out a place where the tenant pays all utilities, does the tenant rent the water heater or does the landlord normally pay for it and keep it in their name?

Landlord is responsible for equipment which is "included" in rent.
Tenant should pay for their actual consumption.

Still confused. Do most landlords include the water heater rental in the rent or do they have the tenant pay for the water heater rental on their own separately from the rent?

If you owned the water heater, the rent would include use of it.
If you want the bill to be in their name, so they pay for usage and rental, your rent needs to account for that.
Depends on the property, # of units etc.
